Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.Active California Physical Therapy license required to start the assignment.BCLS / BLS certification required.Minimum 1 year of recent skilled home health experience. At least 1 year within the last 3 years is required.OASIS experience required for clinicians performing start of care visits.Experience with HCHB electronic medical record required.Comfort with a geriatric caseload.Traveler must have reliable personal transportation. Occasional travel radius may extend up to 50+ miles.Ability to maintain productivity targets: may visit up to 6 patients per day; 30 productivity points standard. Assignment Details Facility Type: Home Health.Shift Name: Days.Shift Details: Monday through Friday, 8:00 AM to 5:00 PM.Patient Ratio/Case Load: Geriatric caseload; may visit up to 6 patients per day.Required Systems/Processes: HCHB and OASIS experience required.Certifications: BCLS/BLS required.License: California PT license required.Traveler transportation: Must have own transportation; routes sometimes grouped to minimize drive time.
