Growing together.  In this RN Clinical Coordinator role, you will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.  There will be travel expectations throughout these counties and region.  If you reside in the state of New York, you'll enjoy the flexibility to work remotely * as you take on some tough challenges.   Primary Responsibilities:Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of carePerform the NYS UAS Assessment in the member's home at least twice per year and as neededIdentify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive servicesManage the care plan throughout the continuum of care as a single point of contactCommunicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all membersAdvocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team  You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current, unrestricted RN license for the state of New York2+ years of relevant clinical work experience1+ years of experience of community case management experience coordinating care for individuals with complex needsExperience in long-term care, home health, hospice, public health or assisted livingProficiency with MS Word, Excel and OutlookNew York state issued ID or ability to obtain one prior to hireAbility to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices  Preferred Qualifications:Behavioral health or clinical degreeExperience with electronic chartingExperience with arranging community resourcesField based work experienceBackground in managing populations with complex medical or behavioral needsProficient in use of UASNY  The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment.
