Job Description:We are seeking a skilled professional to empower individuals in a Day Habilitation setting. As a Community Integration Professional, you will collaborate with teams to ensure the best possible care for those served.For a complete understanding of this opportunity, and what will be required to be a successful applicant, read on.The ideal candidate will have experience identifying members' interests, preferences, skills, strengths, and contributions, planning and coordinating individualized daily and weekly day habilitation schedules.This includes participating in community activities related to hobbies, leisure, personal health, and wellness, cultural, civic, and religious interests, adult learning opportunities, and volunteer opportunities.You will provide services based on unique goals and behavioral care plans, teaching individuals how to accomplish their goals rather than completing tasks for them.A successful candidate will motivate and encourage individuals to participate in their behavioral care plans and reach their goals, understanding when to apply rights-restrictions as determined by the interdisciplinary team.Documentation of progress is crucial, including safe environments and instruction to individuals served regarding practices that help them reach their goals.
