Job DescriptionJob DescriptionSchedule:Have you got the right qualifications and skills for this job Find out below, and hit apply to be considered.•Shift: 9:00 AM - 5:00 PM•Travel: 70% office-based; travel to patient homes (Manhattan, Brooklyn, Bronx, and Queens) for wound care consults as needed•Caseload: 15-20 consults per dayResponsibilities:1.Assess and treat complex wounds, ostomies, pressure injuries, burns, and diabetic foot wounds.2.Collaborate with an interdisciplinary team to develop individualized, evidence-based care plans.3.Provide patient and family education on wound care, infection prevention, and treatment measures.4.Participate in the development and revision of wound and ostomy care policies and protocols.5.Train and evaluate field nurses on wound care techniques and documentation.6.Conduct quality improvement initiatives, case load reviews, and chart audits.7.Ensure compliance with state, federal, and agency policies and procedures.RequirementsQualifications:•License/Certifications:•Active New York State Registered Nurse License•Wound Care Nurse Certification or Wound Ostomy Care Nurse Certification•BCLS Certification•Education: Bachelor's Degree in Nursing (required)•Experience:•Minimum 2 years in a Certified Home Health Agency or post-acute care setting•2 years of medical/surgical experience•2 years of public/community health experience•Familiarity with performance improvement initiatives and relevant xijylhu software applicationsSkills and Technical Requirements:•Knowledge of EPIC H2O, Microsoft Office Suite, and PI applications•Experience with wound care equipment, including Varisource/HDR and CT Simulator•Strong verbal, written, organizational, and analytical skillsRequirements:•Flu vaccine required (medical/religious exemptions accepted)•COVID-19 vaccine not requiredBenefitsCompensation:•Pay Rate: $91/hour
