In this role, you will be the DON primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.ResponsibilitiesWho has held a position of a supervisor in home health care for at least two yrs and able to prepare a comprehensive care plan. xijylhu Coordinate and supervise other colleagues to assess, implement and evaluate staff performanceAble to do admin work and also comply with accreditation processDocument any changes in patient conditions and able to do incident report documentationMaintain accurate records of patient care and discuss observations with the management QualificationsCurrently licensed Registered Nurse (RN) in state of employmentPrevious experience as a Home Care RN with two YRS Experience would be preferredExcellent in communication skills, interpersonal and client care skillsValid drivers license ,Physicals, Xray, a copy of previous background checks will be a plusFirst aid/CPR certification /malpractice insurance is requiredFlexible work from home options available.
