This role focuses on patient assessment, care planning, treatment implementation, and collaboration with interdisciplinary teams to ensure safe, effective dialysis care.ResponsibilitiesAssess, monitor, and evaluate patients throughout dialysis treatmentsDevelop, implement, and document individualized nursing care plansPerform therapeutic nursing interventions and manage overall patient careInitiate and update assessments based on patient needs, physician orders, and clinical findingsCoordinate care with physicians, PCTs, and interdisciplinary team membersDocument nursing care in accordance with professional standards and clinical prioritiesAdvocate for and apply evidence-based nursing practicesDeliver patient-centered care aligned with professional nursing standardsPerform additional duties as assignedRequired Skills & ExperienceMinimum 1 year of outpatient dialysis experienceMinimum 1 year of ICH dialysis experienceProficiency with Fresenius T machines and portable RO systemsPreferred Skills & ExperiencePrior telemetry and chronic hemodialysis experienceAbility to manage:A 3-patient assignment for initiation and discontinuation of treatmentA 9-patient assignment with support from 2 PCTs for medication administration and assessmentsEducationRequired: Associate Degree in Nursing (ASN)Preferred: Bachelor of Science in Nursing (BSN)Certifications & LicensureActive New Jersey RN License or Multistate Compact License (Required)BLS Certification (AHA Required)For more details reach at or Call / Text at 516-587-6677.About Navitas Healthcare, LLC: It is a certified WBENC and one of the fastest-growing healthcare staffing firms in the US providing Medical, Clinical and Non-Clinical services to numerous hospitals.
