This part-time, flexible role offers the opportunity to work in a rewarding environment where your skills make a real impact.In this pivotal position, you will assist the Medical Director with face-to-face Medicare recertification evaluations, conduct home visits, and communicate closely with the Case Manager to ensure optimal patient outcomes. The role may include rounding in inpatient units, educating clinical staff, and providing care in diverse settings such as SNFs, care facilities, or patients' homes. The position is home-based with flexible scheduling, accommodating your availability, whether full or half days.Required Skills:Current Nurse Practitioner license with Palliative Care CertificationPediatric experience is essentialStrong clinical assessment and communication skillsAbility to work independently and as part of a multidisciplinary teamExcellent organizational skills and attention to detailNice to Have Skills:Prior experience in hospice, palliative, or home health careExperience with Medicare recertification processBilingual abilities in English and Spanish or other languagesProficiency with electronic health records (EHR) systemsPreferred Education and Experience:Master's degree in Nursing (MSN or DNP)Minimum of 2 years of Palliative or Hospice care experiencePediatric healthcare backgroundOther Requirements:Valid driver's license and reliable transportation for home visitsAbility to navigate diverse patient environments, including SNFs and homesTravel reimbursements provided as applicableWillingness to work flexible hours, including some weekends as neededHourly Pay Rate: $78.39VHS offers competitive pay packages, weekly pay via direct deposit, comprehensive medical benefits (W-2), a robust referral bonus program, and a dedicated support team committed to your success.
