Additional Information About the RoleMake sure to read the full description below, and please apply immediately if you are confident you meet all the requirements.Up to a $1500 Sign On BonusThis is a 40 hour per week Community Outreach Case Worker positionYou will assist clients by linking them to community resources, such as housing and food assistanceYou will work out in the communities with clients daily (you do not work inside the agency location)Monday - Friday (Evening Shift) - 12 pm to 8:30 pmEvery other weekend rotation Holiday rotation Local travel is required and you will be able to use a company vehicleYou must have a valid CLASS E or CLASS D driver's license required - (if you do not, you must obtain one within two weeks prior to your start date)You will be asked to meet with clients face to faceYou will be asked to meet with clients in their homesYou will be required to connect with all your clients in person on a regular basisRelated work experience with individuals suffering from mental health challenges strongly preferred (NOT REQUIRED)You will be working with individuals that suffer from severe mental health issuesNo caseload Training will take place during your first 90 days and will sufficiently prepare you for this type of work (must be able to learn new things quickly and have a strong ability to use technology)May be eligible for loan repayment assistance from the National Health Service Corps (NHSC).  The link to the program is  Window to apply is through March 31.   OverviewBenefits Summary. xijylhu *Not all benefits apply to all jobsThe above information on this description has been designed to indicate the general nature and level of work performed by employees in this position.
