You will be responsible for providing our patients with personalized, coordinated health care that improves patient outcomes and prevents re hospitalization.As one of our advance practice providers, you will exercise your independent judgement to treat patients with acute and chronic conditions, consulting with a supervising physician as appropriate. You will work closely with our facility partners and health plan leadership to Implement our Treat in Place Protocols in compliance with all federal and state regulations.TruHealth is a division of American Health Partners, a multifaceted company with operations in multiple states. This position gives you the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers.
