You will play a critical role in managing both chronic and acute conditions, participating in transitional care planning, and supporting the facility?s quality improvement initiatives.Core Responsibilities:Collaborate with Trumen physicians and facility medical directors to manage patient careConduct comprehensive clinical assessments and manage treatment plansParticipate in transitional care planning and prevent avoidable hospital readmissionsReconcile medications and provide patient educationFoster positive working relationships with facility leadership, clinical staff, and care coordinatorsParticipate in Trumen?s value-based care programs by tracking patient outcomes and supporting quality improvement initiativesServe as a liaison between Trumen and assigned skilled nursing facilities (SNFs)Maintain accurate and timely documentation within the EMRBe present on-site during regular business hours, depending on availabilityQualifications:Active Nurse Practitioner or Physician Assistant license in the state of TexasMinimum of 1 year experience as an APP, preferably in a post-acute, SNF, or geriatric care settingAbility to work independently while effectively collaborating with the care teamStrong interpersonal skills and a passion for caring for complex patientsReliable transportation to the facilityAbout Trumen Physicians + Associates:Founded in 2010, Trumen is a physician-led, provider-centric organization focused on improving outcomes for patients in post-acute and transitional care settings.
