Collaborates with a physician(s) and other clinicians in providing a full scope of patient care. The focus is on preventive health or the management of acute and/or stable chronic conditions. Performs medical services that have been specifically authorized and directed by the credentialing body and the certifying organization through which they hold licensure.Position SummaryThe APP provides health care services to patients working in collaboration with a licensed physician member of the medical staff. Healthcare services provided include care during acute and chronic phases of illness, education, and counseling of individuals and families, health promotion and prevention, and referral to other healthcare providers and community resources when appropriate. Service line was started in 2018, to see all Advanced Heart Failure patients, pre- and post-transplant, LVAD, inotrope, and admit patients with Heart Failure that have other acute medical conditions.Minimum QualificationsRequired: Master's of Science Degree or DNP in Nursing or Master's in Physician Assistant Science.
