Job Purpose: Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members.Apply below after reading through all the details and supporting information regarding this job opportunity.Duties: Triage Support: Triage patient phone calls and efficiently communicate patient needs to the NP/MD providing triage coverage.Laboratory Review & Follow Up: Collaborate with the triage nurse practitioner to review outpatient laboratory results and communicate any resulting medication changes or updates to the plan of care to patients and caregivers.Clinic Support: Provide support during clinic visits and follow-up care in collaboration with physicians (MD) and/or nurse practitioners (NP).Responsibilities include: Conducting medication reconciliation Performing review of systems Coordinating follow up with providers (including lab review and medication adjustments) Communicating updates to the care plan with patients and/or caregivers Patient & Family Education: Provide health education to patients and families as part of ongoing clinical care.Administrative Responsibilities: Assist with tasks such as: Managing incoming and outgoing faxes Completing school forms and dental clearance letters Processing prior authorizations Other related program support duties as assigned Skills/Qualifications: BLS, BSN Pre-Transplant/Candidates Absolute contraindications to transplant Relative contraindications to transplant Assists with organ offers, acceptance, and recipient preparation Pre-transplant evaluation and assessment Pre/Post transplant care of: *- Preferred Heart Kidney* Liver* Lung Post-Transplant/Recipients Monitors post-transplant patient progress Teaches medication adherence and post-op care plans Regulatory & Administrative Compliance Ability to remove transplant xijylhu recipient from the list per UNOS regulations Tracks patient outcomes and reports per regulatory standards Clinical assessment and skills Recognition and management of transplant complications (infection, rejection, etc.) Medication Administration Coordinates medication adjustments post-transplant with provider Reviews pre-transplant medication regimens for contraindications Professional Knowledge and Skills Coordinate all patient care needs before, during and after transplantation Schedules and tracks pre- and post-transplant testing Understanding of post-transplant health maintenance and long-term care, Waitlist criteria, Pediatrics
