Roles & ResponsibilitiesManage an active panel of SEARHC patients by coordinating care and follow-upCollaborate with providers and interdisciplinary team members to set individualized medical goalsCoordinate multi-disciplinary care conferences for high-risk or complex patientsPerform patient assessment, documentation, education, and necessary nursing interventionsTrack and manage referrals, waitlists, and outcomes across SEARHC and outside facilitiesServe as primary link for case managers and discharge coordinators to ensure continuity of careFacilitate specialty clinic preparation and assist with procedures as neededAct as primary case manager for OB patients including intake, education, and postpartum follow-upSupport telehealth visits by coordinating VTC connections and acting as nursing staff for those visitsParticipate in triage, emergency response, and ongoing development of the case management role QualificationsActive, full, unrestricted Registered Nurse license in Alaska required at time of submittalMinimum 2 years of recent case management experience within the last 4 years requiredAlternative pathways accepted: at least 1 year general nursing with 6 months clinic experience at SEARHC OR 2 years general nursing with 1 year ambulatory or 1 year case managementBasic Life Support (BLS) certification preferredNational Case Management certification preferredProof of flu vaccination and ability to meet annual infection control competenciesAbility to complete fingerprint/background processing prior to start (fingerprint appointment and timely mailing required)Capacity to stand for long periods, lift/move patients or equipment, and work in a fast-paced clinical environment EducationGraduate of an accredited school of nursing requiredBachelor's degree in Nursing preferredHigh school diploma or equivalent required Knowledge & SkillsSkilled in patient assessment, triage, and clinical documentationStrong care coordination, referral tracking, and follow-up skillsExperience using EHR systems and reviewing EHR reminders and triggersKnowledge of safety and infection control principlesExcellent oral and written communication and conflict resolution skillsAbility to work independently and as part of a multi-disciplinary teamFamiliarity with telehealth (VTC) workflows and specialty clinic supportCustomer service focus and advocacy for patient-centered care BenefitsSupportive team that values YOU and your clinical expertiseTravel reimbursement and assistance to make your move easierLicensure assistance and reimbursement for required credentialsHelp with housing arrangements or housing stipend optionsProfessional development and opportunities to expand case management skillsPaid time off to recharge and maintain work-life balanceAccess to workplace wellness and infection control programsA role with meaningful patient impact and strong interdisciplinary collaboration Travel Perks: Explore Juneau, AK Juneau is an outdoor enthusiast's playground.
