This role includes completing evaluations, creating individualized plans of care, and providing evidence based treatment focused on ADLs, functional mobility and transfers, upper extremity strength and coordination, balance, cognition and safety awareness, and adaptive equipment training. You will collaborate with an interdisciplinary team, educate patients and caregivers, support discharge planning, and complete timely, accurate documentation to drive strong functional outcomes.Active Vermont OT license required (or eligible to obtain). Experience in rehab, SNF, hospital, outpatient, or home health preferred. Strong communication and time management skills required, with comfort managing a daily caseload. EMR documentation experience is a plus.
