*Qualifications:** Physical Therapy License or Physical Therapy Assistant License (Required)* CPR Certification (Preferred)*Pay:** Physical Therapist: $42.00 - $60.00 per hour DOE* Physical Therapy Assistant: $30.00 - $35.00 per hour DOE*Primary Responsibilities*:* Provides physical therapy to home care patients and case management when indicated.* Responsible for timely and accurate communication and documentation relative to physical therapy services.* Promotes and develops physical therapy services in homecare*.**Specific Job Duties - includes, but not limited to:** Evaluate and treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment.* Performs initial start of care assessment and all related forms.* Develop, revise and, execute an appropriate care plan for restorative and maintenance patients.* Collaborate with physicians, case managers, nurses, occupational therapists, physical therapy assistants, home health aides, and office staff to ensure consistent levels of care.* Monitor patient progress and document accordingly.* Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment.*About Us:*Hiawatha HomeCare is a family-owned agency in Red Wing MN.
