A Legacy of Care, A Future of Opportunity!We’re looking for an experienced and detail-oriented MDS RN to lead the coordination and completion of the MDS/RAI process and support resident-centered care planning.RESPONSIBILITIES:Coordinate and complete accurate and timely MDS assessments and care plans in accordance with federal and state regulations.Work collaboratively with the interdisciplinary team to ensure resident needs are met and care plans are comprehensive and updated.Conduct assessments to identify functional levels, medical needs, and psychosocial status.Monitor documentation to ensure compliance with Medicare/Medicaid and other payor requirements.Participate in quality improvement initiatives and assist in survey preparedness and responses.Provide education and support to staff regarding MDS documentation and care planning processes.QUALIFICATIONS:Valid Registered Nurse (RN) license in California.Minimum of 1–2 years of MDS experience in a long-term care/skilled nursing facility setting.Strong understanding of PDPM, RAI process, care planning, and Medicare guidelines.Excellent organizational, communication, and clinical documentation skills.Knowledge of electronic health records (PointClickCare experience preferred).WHAT WE OFFER: We value our team members and provide a supportive and enriching work environment.
