*Alternative Family Living (AFL) Host Family Opportunity**Provide a Safe, Supportive Home for an Individual with IDD*A Caring Heart Case Management, Inc. is seeking a compassionate and committed *AFL Host Family* to open their home and provide a safe, inclusive living environment for an individual with intellectual and developmental disabilities (IDD). This is a meaningful opportunity to make a lasting impact by offering stability, acceptance, and a true sense of belonging.*Role Overview*As an AFL Host Family, you will welcome an individual with IDD into your residence and support their daily living needs while treating them as part of your family. This includes fostering personal growth, encouraging independence, and ensuring that all medical, developmental, and environmental needs are met within the home.*Key Expectations** Provide a stable and nurturing home environment* Integrate the individual into daily family life and routines* Support medical, therapeutic, and developmental needs as required* Maintain consistent communication with agency staff and care team members* Ensure safety, dignity, and respect at all times*Home Requirements: *To meet the needs of the incoming resident, the AFL Host Family must have:* A *wheelchair-accessible home*, including an existing ramp or willingness to install one* *Barrier-free mobility* within the residence to allow unrestricted wheelchair navigation* *Sufficient space* to accommodate medical equipment and adaptive devices as needed* A clean, safe, and supportive home environment*Ideal Candidates** Families with a strong desire to make a meaningful difference* Individuals experienced with caregiving or the IDD population (preferred, not required)* Households with patience, compassion, and a commitment to inclusion* Families ready to welcome someone into their home as part of their daily lifeIf you are ready to open your home and heart, we would welcome the opportunity to partner with you.*To learn more or to express interest, please contact our AFL Coordinator.**About This Position:** AFL or Alternative Family Living is a type of Residential Supports available to NC Innovations consumers.* An AFL provider is an individual or couple who chooses to provide Residential Supports in their home. The AFL site (home) must be the primary residence of the AFL provider. AFL is a comprehensive service individually designed to meet the needs of the consumer.* An AFL provider receives payment for the services and support provided to the consumer. AFL providers also receive reimbursement for room and board expenses of the consumer through a separate funding source.* Habilitative services focus on the individual's ability to reside as independently as possible in the community. The individual is assisted with acquiring, improving and retaining skills in self-help, general household management, meal preparation, personal finance management, socialization and other adaptive areas.* Assistance in activities of daily living is provided per the individual's needs. Full participation in home life and community are also encouraged and supported. Please indicate whether you are physically able to push, pull, and lift up to 25 pounds on a routine basis, and up to 100 pounds on an occasional basis. In your response, please describe any relevant caregiving, mobility-assistance, or physically demanding tasks you have previously performed that demonstrate your ability to meet these requirements safely.* Home Accessibility Requirements - A wheelchair-accessible home is required for this placement. Please confirm whether your home currently includes a wheelchair ramp, wide enough doorways, and barrier-free pathways throughout all primary living areas. If your home is not yet fully accessible, please explain your willingness to install a ramp, make reasonable modifications, or adjust your layout to ensure the consumer can navigate safely and independently. Include any steps you have already taken or plan to take to meet accessibility standards.* Experience with Wheelchair Users and Daily Support - Please describe any experience you have working with or caring for individuals who use a wheelchair. Include specifics such as assisting with transfers, managing daily hygiene routines, supporting mobility, monitoring safety, or working with adaptive equipment. If you do not have previous experience, please describe your comfort level with learning these skills and how you would adapt to supporting these daily needs.* Training and Compliance Requirements - AFL Host Families are required to complete mandatory training, follow all agency procedures, and participate in ongoing safety and compliance inspections conducted in accordance with NC DHHS regulations. Please confirm your understanding of these requirements and describe your willingness to follow scheduled inspections, maintain required documentation, and participate in ongoing training to ensure the consumer’s health, safety, and well-being.* Household Readiness and Support - Welcoming a consumer into your home requires commitment from the entire household. Please describe how your family members feel about this opportunity. Include information about their readiness to participate, their comfort level with individuals with IDD, and how your household will work together to provide a safe, inclusive, and respectful home environment.* Understanding of Role and Home Commitment - Please describe your understanding of the AFL Host Family role. Specifically, confirm that you recognize this position requires you to provide dedicated bedroom space within your home, integrate our consumer into your daily family life, and maintain a safe, stable, and welcoming environment where they can live long-term as part of your household.
