Job DescriptionThis is a telework position that requires days working in the Louisville office. Qualified candidates must have reliable transportation. This position is assigned to the Salt River Trail Region. Candidates must reside within communicable distance to the Louisville office. The schedule is Monday-Friday, 8:00am-5:00pm EST with flexibility to work beyond core business hours required, as we are serving the needs of children and families that may require working after school, after work, etc.The Case Management Associate supports comprehensive coordination of services including health risk questionnaires, screenings, and supporting the case management staff. This position utilizes critical thinking and judgement to support the case management process, promotes and supports the quality effectiveness of healthcare services.Evaluation of Members:- Through the use of care management assessments and information/data review, recommends an approach to resolving care needs maintaining optimal health and well-being by evaluating member's benefit plan and available internal and external programs/services.- Coordinates and implements Wellness care plan activities and monitors member care needs.- Identifies and escalates quality of care issues through established channels.Pay RangeThe typical pay range for this role is:Minimum: 18.50Maximum: 33.65Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications- Must live in Jefferson County KY - 2-4 years' experience as a medical assistant, office assistant- 2+ year(s) computer literacy and demonstrated proficiency is required in order to navigate through internal/external computer systems, and MS Office Suite applications, including Word and Excel.-Residing within commutable distance to the Louisville, KY office- Effective communication, telephonic and organization skills.- Ability to effectively participate in a multi-disciplinary team including internal and external participantsPreferred Qualifications- Managed Care experience preferred.-Bilingual preferred-Foster care/adoption/DJJ experience preferredEducationHigh school diploma or G.E.D.Business OverviewBring your heart to CVS Health Every one of us at CVS Health shares a single, clear purpose: Bringing our heart to every moment of your health.
