Job DescriptionSummary/Mission/GoalsThe Goal of the program is to remove barriers that our members face due to Social Determinants of Health issues, access to care and how to access needed services for their health care needs. This will be done in a linguistically, culturally appropriate manner. Position Summary/MissionMust live in Kings County and be able to travel throughout Kings with trips into Queens & Bronx for community eventsThe Community Health Educator (CHE) works directly with members in the community to promote wellness and teach the population how to make better and healthier life decisions. They are professionally prepared and possess knowledge and skills based upon theories and research to promote health education behavior change in individuals and populations. CHEs may share the same community, personal, cultural, linguistic, health condition, and other characteristics of those they serve. Fundamental Components The Community Health Educator will serve as lead on this project while reporting to the Principal Clinical Lead in Community Care. The CHE will create written materials as requested for education and training on health care topics Provides group and individual level education on chronic health issues Creates public and private partnerships in the Kings, Queens and Bronx communities for the purpose of building partnerships and resources for our members Collaborates with CBOs and Providers (FQHC, Dept of Health, traditional providers) Reinforcement of health care messaging in linguistically appropriate language Service coordination and navigation Assist members with program application completion that addresses Social Determinant of Health issues and barriers Educate members on Life Skills such as budgeting, food shopping, food preparation Teach members how to manage existing health care conditions using a behavior modification approach. Educate members on Healthy Living, Physical and mental health well begin, Tobacco free drugs, alcohol. Assist the member with referrals for services in the community Document work with member in a database system and review as a team Responsible for aggregate data reporting for the teamPay RangeThe typical pay range for this role is:Minimum: 19.50Maximum: 40.10Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls.
