Position Highlights Department:Center for Chronic Care ManagementHours:Part-Time/20 hoursShift:Days, 1st Shift, no weekends or holidaysPosition Summary: TheLCSW Care Manageris responsible for focusing on providing support in the management of anxiety and depression for patients dealing with chronic disease as well as support and education for patients who wish to quit tobacco addiction.
