Maintain accurate and timely documentation of patient care, ensuring quality, continuity, and compliance with regulatory and third-party payer requirements. Educate patients and families on medications, care techniques, safety, and self-care based on age, condition, and readiness to learn. Supervise and guide support staff (HHA/STNA/LVN) and coordinate referrals to other healthcare agencies as needed. Perform on-call duties, participate in interdisciplinary team meetings, and communicate effectively with all parties involved in patient care.Required Knowledge, Skills & Abilities: Effectively assesses and responds to patient and family needs in diverse care settings. Demonstrates strong communication, organizational, and critical nursing skills, including wound care, phlebotomy, and IV therapy. Competent in using and maintaining medical equipment, catheters, and administering respiratory therapy. Maintains current licensure, confidentiality, and meets continuing education requirements.
