Our interdisciplinary services include skilled nursing, physical therapy, occupational therapy, speech-language pathology, medical social work, and medical supplies as needed.We are seeking a part-time, 32-hour Speech-Language Pathologist to join our dynamic "Mayflower" team. Your time will be spent in Plymouth County.Enjoy the freedom to create your own schedule while providing dedicated 1:1 care-because your work-life balance is essential. The typical schedule is Monday through Friday, with an anticipated caseload of approximately 4-5 visits per day, plus time allotted for documentation, visit planning, chart review, and care coordination.We seek candidates who bring not only the required clinical expertise but also positive energy, emotional intelligence, and a genuine passion for empowering patients through communication and swallowing rehabilitation. Join a team where your clinical skill is matched by purpose, and your commitment to enhancing lives is supported every step of the way!Job SummaryProvide speech therapy services to patients in their place of residence who have impairments, functional limitations or disabilities resulting from injury, disease or other causes. Patients may range in age from birth to over 100 year. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs. Manage technical, environmental and financial resources.QualificationsKey Responsibilities* Age appropriate evaluation is completed that reflects patient history, relevant systems review and appropriate tests and measures and reported to primary clinician. * Plan of treatment is reassessed at least every 2 weeks to patient, primary caregivers and interdisciplinary team; plan is modified and communicated as appropriate to meet discharge goals. * Physician orders are current and consistent with plan of care and documented in the medical record. * Patient schedules are submitted to the rehab scheduler by end of day Thursday. * Home Health aides will be oriented and supervised in accordance with agency and state guidelines. * Patient caseloads will be managed in accordance with agency, state and professional guidelines. * Documentation is complete and exported timely according to agency standards. * POC devices are updated weekly and as directed * Verification of Payor information on admission/eval is completed within agency and regulatory guidelines. * Documentation to support Managed Care authorizations/DME requirements is maintained/transmitted within designated time frames. * Visit schedule is consistent with authorizations by Managed Care payors. * Casemix Weight meets Agency Standards. * Utilization/Caseload is discussed with Rehab Manager on an ongoing basis and changes are made toward stated goals. * Reportable outcomes meet Agency goals. * Verification of Payor information on admission/eval is completed within agency and regulatory guidelines. * Realistic, objective, time oriented goals for patient performance are identified based on examination findings. * Treatment and discharge plans are selected based on examination findings and patient goals. * Patient is informed of his/her weekly schedule and any other changes to their schedule as necessary.Education/Degree RequirementsLicensure, Certifications or RegistrationRequired: Registered with the Board of Registration in Massachusetts.Experience* Two years experience in acute care, rehab or homecare as a Speech Therapist * Registered with the Board of Registration in the State of Massachusetts * Proficient computer skills with ability to learn new software applications * Experience working in an interdisciplinary team. * Strong verbal, written and electronic communications skills * Excellent interpersonal and customer service skills with ability to work with a diverse population * Strong organizational skills with ability to prioritize assignmentsStrong organizational skills with ability to prioritize assignmentsAdditional Job Details (if applicable)Remote TypeHybridWork LocationOne Scobee CircleScheduled Weekly Hours32Employee TypeRegularWork ShiftDay (United States of America)Pay Range$71,697.06 - $138,361.60/AnnualGradeHCCLPR100At Mass General Brigham, we believe in recognizing and rewarding the unique value each team member brings to our organization.
