PASCO seeks a skilled, compassionate nurse coordinator to provide direct care, oversight, and education to our IDD clients.What You’ll DoDeliver care coordination, clinical oversight for IDD clientsDevelop and update care plansConduct quarterly/annual visits to review care plans, skilled/unskilled tasks, medication managementMaintain demonstrated competency in skilled nursing tasks as it is a key component of caregiver education and ongoing trainingTrain/support Direct Service Providers (DSPs) on medical tasksComplete nursing delegation, ensure regulatory complianceUse Therap software for records (experience preferred)QualificationsColorado RN license (required; ASN or BSN — both accepted)BLS/First Aid certification (required)Willingness to travel across listed territoryStrong communication, teaching, documentation skillsPreferred SkillsBSN (preferred, not required)1+ year acute care, IDD or home health experience (plus, not required)Therap software familiarity (plus)Bilingual Spanish (highly desired, not required)PASCO is an Equal Opportunity Employer.
