Job description: The Moxie Health Group LLC is a multi-specialty practice group specializing in pulmonary, infectious disease, cardiology, and post-acute care. We currently have over 100 providers delivering specialized care to patients in skilled nursing facilities across Florida, Ohio, Tennessee, and now in Pennsylvania and Texas.A core aspect of our program is reducing hospital readmissions, ensuring responsible stewardship, and optimizing patient care within these facilities. Our Corporate Headquarters is in Hallandale Beach Florida.Our Mission Statement: We streamline patient care by ensuring real-time response, quality assurance, accurate medical decision-making, and thorough documentation, thereby improving both patient outcomes and facility efficiency.Locations:We have current openings in the different locations listed below:Athens, TXCrockett, TXRusk, TXHenderson, TXKilgore, TXJob ResponsibilitiesDeliver direct patient care in skilled nursing, rehab, and long-term care settingsPerform health assessments and physical examinationsDiagnose and develop treatment plans tailored to each patientPrescribe medications per Florida guidelines and clinical best practicesComplete documentation and maintain accurate health records (EMR/EHR)Conduct admission and discharge evaluationsCoordinate care with physicians, facility staff, and specialistsPromote preventive health practices and counsel patients on healthy livingEnsure continuity and quality of care with timely interventionsParticipate in facility rounds and contribute to reducing hospital readmissionsEducate patients and families about treatment plans and care routinesComply with infection control standards and regulatory policiesQualificationsActive Florida APRN licenseDEA registration (preferred)BLS and ACLS certificationsActive NPIMinimum of 3 years of nursing experience (Skilled Nursing preferred)Knowledge of post-acute care and geriatric best practicesStrong communication, assessment, and leadership skillsExperience with EMR/EHR systems (Point Click Care/ Matrix)Comfortable working independently in a fast-paced, clinical environmentWorking Hours:5 Days a week with weekend availability (Pulmonary Specialty)Skills:The ideal candidate should have experience or knowledge in the following areas:Performing physical exams and patient observationsBehavioral health assessmentSetting up and monitoring medical equipmentCommunicating test results to patients and their familiesRecording patient medical historiesFamily planning servicesPost-acute care experience (Preferred)Senior care practicesMedical-surgical nursing techniquesStrong anatomy knowledgeJob Type: Full-TimeBenefits:Census PlanningOngoing EducationCompetitive Pay StructureMoxie Link (Quality Assurance Team)Paid TrainingFacility ShadowingMalpractice CoverageMedical Specialty:Physical & Rehabilitation MedicinePulmonologyAcute Care Job Type: Full-time Experience:Pulmonology: 2 years (Required)Nursing home: 1 year (Required)Infectious disease care: 1 year (Required)License/Certification:NPI License (Required)Shift availability:Day Shift (Required) Work Location: In person
