A well-established healthcare organization is seeking a compassionate Field Nurse Practitioner to join their home care team. Covering the Manhattan and Bronx territories, this role involves providing care to patients in their homes. Responsibilities: Provide comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self-management education & tools to better manage patient's multiple conditionsProvide and coordinate primary care services to an assigned the at-risk panel of geriatric patients who reside in the communityEnsure development of care planning and access to preventive care services in accordance with member and plan health outcome goals and objectivesAct as a clinical resource to the member, family, and interdisciplinary teamQualifications: Active ANP, FNP, or GNP license in the state of New YorkAt least one year of Nurse Practitioner experience Previous experience with home care and/or geriatrics
