Seeking Palliative Care Nurse Practitioner to join an expanding Inpatient Palliative Care (Hospice) team. 100% inpatient rounding on hospice patients. Monday - Friday. Support 6 patients per day. 5 day workweek, 8am - 5pm, rotating on-call. Practice autonomously. Assess, plan, implement, and evaluate care needs. Symptom management, chronic disease management, goals of care, advanced care planning. EPIC EMR Join a non-profit tertiary referral center with 350 physicians. 2 hospitals with 420-bed facility. Level II Trauma center. DaVinci robotics. Academic affiliation of Osteopathic Medicine.
