As a key member of our wound care team, you will: Conduct thorough assessments and develop personalized treatment plans that address underlying health issues and co-morbidities Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities Lead wound assessments, treatment planning, and advanced procedures Harness cutting-edge tech and evidence-based protocols to accelerate healing Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and families Keep your charting sharp and timely in our customized EMR Help drive quality initiatives and support our mission of raising the bar in post-acute careQualifications:APRN Full Practice Authority (FPA)Active Illinois (IL) Nurse Practitioner license (board-eligible not accepted)CDS and DEA licenses (must be obtained by end of training period)At least 2 years of wound care experiencePrevious experience in SNFs, long-term care, or rehabilitation facilitiesWillingness to travel to designated partner facilities in the Chicago areaComfortable with EMR, mobile, and laptop documentationStrong interpersonal skills collaborative, communicative, and adaptableAs we grow, you'll be a part of a supportive, forward-moving company that empowers Providers to do what is best for their patients, every time.
