Hybrid Nurse Practitioner Primary/Palliative Care Clarke County, AL A fast-growing, patient-focused healthcare organization is seeking an Advanced Practice Provider (NP or PA) to provide in-home palliative care for patients with chronic and complex conditions. This role is field-based with a hybrid schedule, allowing providers to spend most of their time in patients' homes while dedicating select days to remote work for charting, telehealth, and administrative tasks.
