Provider must take an Epic training prior to start - can fill out waiver if they have years of experience.2. Orientation prior to start is requiredWithin 30 days of start:1. Provider must have a physical and 5 panel drug screen (not reimburse-able) Submission Requirements Board Certified - REQUIRE BLS, ACLS, PALS - REQUIRE Active MI license (not accepting IMLC or Compact) - REQUIRE Self-Query NPDB (Should be pulled within the month) REQUIRED at time of presentation.
