Growing together.Primary Responsibilities:Screen, assess and triage primary care patients for behavioral health disorders, symptoms and conditions including substance use disorders, at assigned clinical site(s)Provide crisis intervention and stabilization for patients presenting to their primary care office at risk or in need of immediate careAnticipate and address gaps in access to Behavioral Health (BH) services by providing interim, back-up or transitional care/treatment and patient follow-up by telephone or in person with suicidal, high need or high-risk patients to monitor and manage risk and safety until they are able to connect with a treatment resource/provider, or while they are in treatmentDevelop triage planning for patients requiring behavioral health treatment with community partners outside of the organizationCollaborate with and provide direction to primary care providers on the ongoing care of patients with mild to moderate behavioral health conditions who can safely and effectively be managed by the primary care physician (PCP)/advanced practitioner (AP) including follow-up (by telephone or in person) as neededAugments patient engagement in BH care by providing short behavioral interventions such as BA- behavioral activation; MI - motivational interviewing, etc. Facilitate referral to treatment, including linking patient back to PCP, to an organization integrated behavioral health provider or to a community partner for treatmentImplement treatment plans, during and/or after course of treatment with integrated behavioral health providerFollow up with patients (by telephone or in person) to determine what variables such as depression, substance abuse, etc., may be affecting patient's ability to attend appointments and/or comply with treatmentWork within and contribute to an integrated approach to Behavioral Health care and communication, working collaboratively and professionally with the primary care team, integrated behavioral health provider and other organization Behavioral Health colleagues, care management team, psychiatric prescriber team, and substance use management teamPresent cases to psychiatric prescribers for recommendations to primary care providers for prescribingMaintain open access for patients in immediate need, for warms hand offs from primary care providers and to be available for consultation on BH related concernsDevelop and complete relapse prevention self-management plan with patients as needed.Manage patient registry lists and follow up with patients to monitor clinical outcomes and risk; consult with other providers regarding patient's return to care and/or possible changes in treatment planProvides and participates in education presentations, case reviews and other department meetings as required or directed. Enhances professional growth and development through educational programs, seminars, etc.Supervised by integrated behavioral health providerMaintains patient, staff and institutional confidentialityComplies with health and safety requirements and with regulatory agencies such as DPH, etc.Complies with established departmental policies, procedures, and objectivesPerforms other similar and related duties as required or directedRegular, reliable and predicable attendance is requiredYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Undergraduate degree or equivalent experienceMaster's degree from and accredited school and active, unrestricted licensure as an LICSW or LMHC to practice independently in the Commonwealth of Massachusetts and 1+ years of experience in a Behavioral Health primary care integrated settingPreferred Qualifications:Primary Care backgroundCrisis Management backgroundPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $60,200 to $107,400 annually based on full-time employment.
