Description:A mission-driven Federally Qualified Health Center (FQHC) in Connecticut is seeking a Family Nurse Practitioner (APRN) to provide comprehensive primary care to patients across the lifespan, from pediatrics through geriatrics. This role is ideal for a clinically strong, team-oriented provider who is passionate about delivering high-quality, accessible care in a collaborative outpatient setting. The Nurse Practitioner will deliver direct patient care while working closely with physicians, nurses, medical assistants, case managers, and support staff to ensure coordinated, patient-centered care. This position follows all applicable Connecticut state practice requirements, including physician collaboration for newer APRNs.
