Description:A mission-driven Federally Qualified Health Center (FQHC) in Connecticut is seeking a Family Nurse Practitioner (APRN) to provide comprehensive primary care to patients across the lifespan, from pediatrics through geriatrics. This role is ideal for a clinically strong, team-oriented provider who is passionate about delivering high-quality, accessible care in a collaborative outpatient setting. The Nurse Practitioner will deliver direct patient care while working closely with physicians, nurses, medical assistants, case managers, and support staff to ensure coordinated, patient-centered care. This position follows all applicable Connecticut state practice requirements, including physician collaboration for newer APRNs. Job ResponsibilitiesProvide comprehensive primary care services within the scope of APRN licensureConduct patient assessments, including medical histories, physical exams, and diagnostic evaluationsDiagnose and treat acute and chronic conditions across the lifespanOrder, interpret, and follow up on laboratory and diagnostic test resultsCollaborate with physicians and the interdisciplinary care team to develop and implement patient care plansRefer patients to specialists and community resources as clinically appropriateParticipate in quality improvement initiatives, professional practice meetings, and staff meetingsEngage in community-based services such as outreach events and health fairs as neededMaintain accurate, timely documentation in the medical recordSupport and advocate for community health programs and population health initiatives QualificationsActive APRN licensure in the State of Connecticut (or ability to obtain)Family Nurse Practitioner certification requiredEligible to practice collaborative care per Connecticut state regulationsExperience in primary care or community health preferredStrong clinical assessment, diagnostic, and patient management skillsAbility to work effectively in a team-based, mission-driven environmentCommitment to professional development and continuing education Schedule & Practice EnvironmentOutpatient, community-based primary care settingTeam-based model with strong clinical and operational supportOpportunity to serve diverse and underserved patient populationsParticipation in community outreach activities as part of the role Skills & Focus AreasFamily Medicine / Primary CareChronic Disease ManagementPreventive CareTeam-Based CareCommunity HealthPatient EducationCare CoordinationFederally Qualified Health Center (FQHC)
