The client will train the right candidate and need someone who can start asap.Job Objective: Provides direct patient care and support to multidisciplinary health team working within the scope of practice of a Nurse Practitioner. Serves as a clinical expert through patient education, interdisciplinary collaboration, and consultation, while maintaining program excellence and professional growth. Responsible for developing comprehensive care plans, coordinating patient care, delivering ongoing treatment while fostering a strong patient-provider relationship. Committed to cost-effective care management, optimizing resource utilization, and enhancing patient experiences through continuous process improvement.
