Director of Clinical Operations (Nurse Practitioner) - Kearny, NJAbout the RoleThis role is responsible for translating clinical strategy into scalable, repeatable, and enforceable operating systems that drive provider productivity, optimize patient retention within preferred partner networks, and ensure consistent care delivery across inpatient, outpatient, home-based, and post-acute settings.The Director of Clinical Operations owns day-to-day operational performance, including workflow design, provider utilization, retention tracking, and cross-setting care coordination. This position serves as the primary execution partner to the Chief Nursing Officer (CNO), ensuring operational discipline, identifying gaps, and implementing effective solutions as the organization scales.Key ResponsibilitiesDevelop and maintain clinical and operational policies to ensure consistency in care delivery, documentation, billing workflows, and regulatory complianceDesign, implement, and manage workflows, referral pathways, triage processes, and scheduling models across all care settingsDrive provider productivity, utilization, and patient retention initiativesQualificationsLicensed Nurse Practitioner (APN) in New Jersey (required)3 5+ years of experience in palliative care, hospice, geriatrics, or complex care settingsACHPN (Advanced Certified Hospice and Palliative Nurse) preferred; required within one year of hireProven experience managing multi-site clinical operationsDemonstrated ability to build and enforce scalable workflowsStrong track record in provider productivity, utilization, and patient retentionExcellent organizational, communication, and problem-solving skillsExperience across hospital, skilled nursing facility (SNF), and home-based care settings preferredProficiency with EMR systems such as Epic Systems, PointClickCare, and Homecare Homebase preferredWhat We OfferCompetitive compensationComprehensive benefits packageLeadership and professional growth opportunitiesSupportive and collaborative work environmentMeaningful work focused on improving quality of life for patients and families
