Conduct comprehensive assessments to establish effective goals for symptom management and end-of-life care. Collaborate with an interdisciplinary team to address the holistic needs of patients. Build and maintain therapeutic relationships through effective communication. Utilize state-of-the-art technology to document services provided skillfully. Qualifications: To qualify, you must be a Registered Nurse with a valid CT RN license, along with: A minimum of one year of RN experience, preferably in Homecare or Hospice.
