Flexible hours, Monday-Friday, No call. A well-established, physician-led group with locations throughout Long Island, is currently seeking to hire a Family Nurse Practitioner. This practice has built a stellar reputation for clinical excellence, patient satisfaction, and multidisciplinary collaboration. 100% outpatient. Key Responsibilities: Serve as a key clinical provider in the delivery of high-quality care. Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management. Refer patients to appropriate specialty services as needed. Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies. Maintain thorough and accurate documentation within the Electronic Medical Record (EMR) system. Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care. Provide clinical oversight to nurses, technologists, and support staff within the practice. Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines. Participate in population health initiatives and support ongoing quality improvement projects. Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
