Facility Name and address: Hocking Valley Community Hospital: 601 SR 664 N Logan, OH 43138 Phone number: 740 380 8332 or 740 380 8232 Licensed Beds: 25 Average Daily Census: 18 Position: Director of Pharmacy plus PIC Skills required: Inpatient experience, IV, 340B Ohio Pharmacist licensure required Dates of Coverage needed: start 3/16/26 until position filled permanently - current DOP's last day March 27th Pharmacy Hours: Monday-Friday 7:30am-6pm, Saturday 8am-4:30pm Coverage Hours needed: Current DOP works 4-10s plus on-call when pharmacy is closed (usually receive 1-2 calls per week) Weekend coverage needed: starting 3/28 - every 4th weekend rotation plus on-call starting Fridays 6pm until Mondays 7:30am Order Entry System: EPIC, experience preferred but can train Site Automation: Pyxis; will train Site Specialty Areas: ED, Infusion, Ortho Staff Size: 1 FT Pharmacist & 2 PRN Tech support: IVs, filling machines but cannot verify orders. Site Background/History: DNV completed in Feb 2026.
