Title: Homecare Registered Nurse Job Code: RN HOMECARE Revised Date: 5/6/2024 Department: Clinical Operations Status: Exempt Job Summary: As a Homecare Registered Nurse, you will play a critical role in delivering compassionate and expert nursing care to patients and their families in a home setting. Your responsibilities will include conducting thorough assessments, creating tailored care plans, and providing support to enhance patients' comfort and quality of life. Duties and Responsibilities: Adhere to the Nurse Practice Act to ensure high standards of care. Conduct initial assessments of home care needs, focusing on patients’ physical, emotional, and mental health. Visit assigned patients regularly to evaluate their conditions and assist with symptom management. Develop and update treatment plans in collaboration with the Interdisciplinary Team (IDT). Deliver direct nursing care, prioritizing patient comfort and quality of life. Maintain open communication with primary physicians, accurately documenting all interactions. Clarify any unclear or unusual physician orders. Coordinate care and services with relevant agencies to streamline patient support. Ensure a safe care environment by managing proper disposal of materials and adhering to universal precautions. Order medications, supplies, and necessary equipment for patient care. Educate caregivers and family members on medication management and care techniques. Administer therapeutic and diagnostic measures competently and safely. Communicate changes in patient conditions to the care team, addressing any concerns holistically. Maintain up-to-date and accurate patient records. Keep the supervisory team informed of all developments affecting patient care. Collaborate with the Volunteer Coordinator to offer additional support to families. Provide oversight and guidance to LPNs and CNAs during patient care. Ensure compliance with recertification protocols for hospice services. Participate actively in staff and IDT meetings. Offer bereavement support when needed. Follow all health and safety regulations in patient care. Partake in on-call coverage as scheduled to ensure continuous care. Perform additional duties as assigned. Required Skills and Abilities: Demonstrate professionalism and a positive image in all interactions. Pass all required background checks and maintain necessary licenses. Have reliable transportation and insurance for travel to patient locations. Qualifications and Experience: Graduation from an accredited nursing program with a current nursing license. Familiarity with current nursing trends and practices. Two years of experience in a healthcare setting. Maintain continuing education in accordance with state licensing requirements. Physical Requirements: Ability to meet physical demands of the position. Pass an in-house physical assessment. Capable of various physical activities, including lifting and moving patients. Work in diverse environments which may include exposure to different conditions.
