Case Manager / Care Coordinator - Level 2 Join Our Team as a Client Advocate & Care Partner We are seeking a compassionate, detail-oriented Case Manager / Care Coordinator - Level 2 to join our Outpatient team. In this role, you will play a vital part in helping clients navigate and access essential behavioral, medical, social, educational, and health-related services. Position Summary The Case Manager / Care Coordinator Level 2 assists clients in identifying needs, accessing resources, and achieving treatment goals through coordinated, person- and family-centered care. This role includes: Coordinating behavioral, medical, social, and educational services Advocating on behalf of clients and families Developing and implementing individualized treatment plans Monitoring progress and adjusting services as needed Collaborating with interdisciplinary providers and community partners Maintaining accurate and timely documentation Education & Licensure Requirements Candidates must meet one of the following qualifications: Bachelor's or Master's degree in a human services-related field (in accordance with the West Virginia Board of Social Work), OR Licensed Psychologist (Master's or Doctoral degree from an accredited program), OR Licensed Registered Nurse (RN) What We're Looking For Strong organizational and communication skills Ability to work collaboratively within a multidisciplinary team Commitment to ethical practice and client-centered care Passion for serving individuals and families in outpatient settings
