This position will split time between Newport & St. Johnsbury Northeastern Vermont Regional Hospital (St. Johnsbury, Vermont) and North Country Hospital (Newport, Vermont), two regional partner critical access hospitals in Vermont's Northeast Kingdom, are jointly seeking a shared Care Transformation Project Manager to lead and support the planning, implementation, and evaluation of care transformation initiatives across both organizations. This shared individual contributor role is designed to provide dedicated capacity, coordination, and expertise to advance strategic transformation priorities while supporting hospital leadership with project management, change management, and cross-organizational alignment. The employer will be Northeastern Vermont Regional Hospital. The Care Transformation Program Manager will work closely with both hospital CEOs and senior leadership teams to translate strategic priorities into actionable plans, ensure disciplined execution, track progress and outcomes, and support continuous learning and adaptation. This position plays a critical role in fostering cultural alignment and bridging strategy and implementation across organizations, departments, and partners. This is a full-time, grant-funded position. This position is grant"'funded, and continued employment is contingent upon ongoing funding. Key Responsibilities Care Transformation Planning & Strategy Support hospital CEOs and leadership teams in defining, refining, and sequencing care transformation priorities. Translate high-level strategic goals into clear transformation plans, road maps, and workplans. Facilitate alignment between hospital-based initiatives and broader system or continuum-of-care transformation efforts. Support development of transformation plans, proposals, and deliverables for internal and external stakeholders. Project & Program Management Lead project management for multiple care transformation initiatives across both hospitals. Develop and maintain project plans, timelines, milestones, risk logs, and decision points. Coordinate cross-functional teams and ensure accountability for deliverables. Track dependencies, identify barriers, and proactively escalate risks or delays to leadership. Assist with designing future-state workflows that improve efficiency, quality, safety, and user experience. Change Management & Implementation Support Support leaders and teams through change management processes, including stakeholder engagement, communication planning, and readiness assessment. Facilitate implementation planning and support operationalization of transformation initiatives. Promote shared understanding of goals, roles, and expectations across teams and organizations. Support problem-solving and adaptation during implementation. Evaluation, Learning, & Reporting Support development of metrics and evaluation approaches to assess progress, outcomes, and impact. Track performance against goals and support data-informed decision-making. Prepare regular updates, dashboards, and briefing materials for CEOs and leadership teams. Capture lessons learned and support continuous improvement across initiatives. Executive & Leadership Support Provide direct analytical, planning, and coordination support to both hospital CEOs. Prepare briefing materials, presentations, and decision memos. Facilitate leadership meetings, working groups, or strategy sessions as needed. Serve as a trusted, neutral resource supporting alignment across organizations. Present plans and progress to senior leadership and boards of directors as requested. Qualifications Required Bachelor's degree in health care administration, public health, business, or a related field (or equivalent experience). 3+ years of project management experience. Experience in health care transformation, program management, strategic planning, or related roles strongly preferred. Demonstrated experience managing complex, multi-stakeholder projects. Strong project management, facilitation, presentation, and organizational skills. Ability to work effectively with senior leaders and cross-functional teams. Excellent written and verbal communication skills. Preferred Master's degree (e.g., MHA, MPH, MBA). Experience working across multiple organizations or in shared service roles. Familiarity with care delivery transformation, value-based care, population health, or health system integration. Formal training or experience in change management methodologies. Reporting & Structure This position will be jointly sponsored by two hospitals. The Care Transformation Project Manager will have a defined reporting relationship and governance structure agreed upon by both organizations. Day-to-day work will be coordinated closely with both hospital CEOs and designated leadership liaisons. Compensation Total annual compensation budget: $90,000. Benefits and employment structure to be determined by the sponsoring organizations.
