Prime Time Healthcare Nursing is seeking a travel nurse RN Care Manager for a travel nursing job in Fort Wayne, Indiana. Job Description & Requirements Specialty: Care Manager Discipline: RN Start Date: 04/06/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Collaborative work with the Care Team. Completion of comprehensive patient assessment including patient status to drive discharge. Driving stay progression Completion of discharge planning in a manner that expedites patients transition to the next level of care. Comprehensive and accurate Epic documentation  Epic ChartingAbility to work independently Ability to identify work that needs to be done and follow through to completion without prompting. Ability to work collaboratively with the care team. Ability to complete a comprehensive patient assessment including patient status to drive discharge. Ability to drive stay progression and complete discharge planning in a manner that expedites patients transition to the next level of care. Ability to document in a manner that is comprehensive, accurate and timely. Minimum of 2 years experience in the role of IP RN Case Manager.State of Indiana RN License or compact Prefer certification in Case ManagementMonday Thru Friday 8-430 1/2 hour Lunch. No call required. No floating50 beds waiting from state to increase beds to 62 bedsBusiness Casual with white lab coat (no jeans) Close toed shoes Facility will accept local travelers  Client can call off up to 24 hours per 13 week contract and there are 8 hours of non-billable orientation.Name of Position Requirements - IN or compact license required. - 2+ years of Inpatient Case manager experience required. - Prefer certification in Case Management. - Prefer inpatient rehab experience.
