Reporting to the Clinical Nurse Manager, you will be responsible for both overseeing team members and delivering RN-level nursing care. Under the guidance of the Executive Director, you will manage the day-to-day operations of the clinic, ensuring that services are delivered effectively and that quality care standards are upheld. Your Day-to-Day Responsibilities: Lead and manage RN, LVN, and MA team members, ensuring they receive the training and support needed to excel. Oversee clinic operations to meet the chronic and acute needs of participants. Collaborate with the Complex Care team to facilitate smooth care transitions for post-discharge patients. Develop staffing plans that accommodate the diverse experiences of team members while addressing participant needs. Engage in direct patient care to ensure optimal care quality, practicing at the RN level. Manage hiring, training, and development of direct reports, adapting responsibilities as business needs change. Required Qualifications: Graduated from an accredited nursing program with a valid RN license; BSN preferred. At least two years of nursing experience in a clinical setting, ideally with elderly or frail populations. One year of supervisory experience, with a focus on mentoring and team development preferred. Experience in case management, care coordination, or a related specialty is preferred. Ability to thrive in a data-driven organization, using reports and analytics to prioritize tasks. Availability to work a flexible schedule, including evenings, weekends, and possible overtime.
