POSITION SUMMARY TruHealth is on the lookout for an advanced practice provider, specifically a nurse practitioner (NP) or physician assistant (PA), with a focus on primary care or internal medicine, preferably with a background in geriatrics. As an advanced practice registered nurse (APRN or NP) or physician assistant (PA) who is passionate about working with the aging population, consider becoming part of the TruHealth team. Our providers typically work within nursing homes or assisted living communities with residents enrolled in specialized Medicare Advantage programs. You will be tasked with delivering personalized, coordinated care aimed at enhancing quality of life and preventing avoidable hospitalizations. In this role, you will utilize your independent judgment to manage acute and chronic patient conditions, consulting with supervising physicians when necessary. You will partner closely with our facility collaborators and health plan leadership to apply our national model of care while adhering to all federal and state regulations. Our NPs and PAs are integral team members who contribute to TruHealth's achievements and assist our partners in elevating the quality of life for their residents. Key responsibilities include: Assessing medical and mental health needs of patients and crafting a proactive care plan that addresses their conditions and barriers to care. Ensuring compliance with all relevant local, state, and federal regulations concerning clinical services. Prescribing medications and ordering necessary lab tests, diagnostic procedures, and consultations. Monitoring patient adherence and response to treatment plans, making adjustments as needed. Collaborating with RN case managers as part of an integrated care team. WORK EXPERIENCE, CREDENTIALS AND EDUCATION Required: FNP, AGNP, AHACNP, or PA license. Graduation from an accredited APRN or PA program. A minimum of 3 years of experience in clinical nursing or rehabilitation within geriatric populations. Familiarity with Electronic Health Records. Proficient in Microsoft Office applications, including Word, Outlook, and Excel. SUPERVISORY RESPONSIBILITIES May require providing training and guidance to facility staff.
