Position Summary: At Eventus WholeHealth, we're committed to improving the quality of life for adults in skilled nursing and assisted living facilities. We are searching for a dedicated Mental Health Social Worker Specialist to become part of our dynamic healthcare team! In this vital role, you will provide recovery-oriented rehabilitation services to residents, addressing various mental health challenges including anxiety, depression, severe mental health issues, and end-of-life matters. As an essential member of our multidisciplinary team, you'll have the chance to make a meaningful difference in our residents' lives by ensuring they receive the highest standard of clinical care. Essential Duties and Responsibilities: Conduct thorough and compassionate assessments of residents, ensuring proper consent to create tailored treatment plans that meet individual needs while guaranteeing all care is necessary. Engage in effective communication with residents' primary care physicians, families, facility staff, and support staff through written and verbal consultations. Meticulously document treatment plans, psychotherapy sessions, and updates in line with Eventus WholeHealth's policies, ensuring all records are submitted within 48 hours of service. Collaborate closely with the Resident Care Coordinator and facility staff to assess care needs and share findings or concerns during your visits to the facility. Maintain open and positive communication with facility staff and administration, providing feedback and guidance on treatment plans and other vital caregiver information. Adhere strictly to Eventus WholeHealth standards, policies, and procedures, as well as legal and ethical guidelines governing healthcare practices. Exemplify professionalism at all times, prioritizing patient confidentiality in all interactions and documentation. Seek guidance from a clinical supervisor when faced with unfamiliar situations or requests regarding resident care. Qualifications/Education: We seek candidates with a Master’s degree in social work and a current license to practice in their respective state. Knowledge: Familiarity with assessment and psychotherapy techniques, with the ability to implement population-specific interventions in accordance with treatment plans. Proficient in using various diagnostic tools, including the latest edition of the DSM. Understanding of the regulations governing psychotherapy practice and a solid knowledge of our organization’s policies. Skills Required: Ability to analyze a wide range of physiological, socioeconomic, behavioural, and emotional data from patients. Competence in evaluating patient issues and providing suitable interventions. Exceptional written and verbal communication abilities. Strong initiative and sound judgment, especially in sensitive situations. Ability to build and maintain effective working relationships. Abilities: Capability to remain calm and effective during emergencies. Willingness to collaborate with a diverse healthcare team. Ability to make informed decisions within the context of a dependent medical practitioner. Focus on continuous quality improvement. Handle confidential information with discretion and professionalism.
