If you are interested in this Home Health RN Case Manager position, please Lindsay at 5867107959 or Home Health RN Case Manager Compensation The pay for this position is $56.14 per hour Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Home Health RN Case Manager Highlights This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs The required availability for this position is Monday–Friday 8:00 AM–5:00 PM with alternating on-call weekends (Sunday–Saturday) Home Health RN Case Manager Responsibilities Perform comprehensive assessments of high-risk members Collaborate with providers to implement individualized care plans Coordinate transitions between care settings Identify appropriate and cost-efficient levels of care Arrange non-medical support services (transportation, housing, etc.) Coordinate with physicians, social workers, discharge planners, and claims teams Maintain detailed clinical, functional, and financial documentation Promote health prevention and intervene quickly when adverse events occur Advocate for patients while maintaining privacy and confidentiality Home Health RN Case Manager Requirements Active Colorado RN license (compact accepted) Associate or Bachelor’s degree in Nursing Case Management certification Ability to analyze complex medical information and make clinical decisions Must have reliable vehicle and auto insurance Must meet productivity requirement of 30 points per week Homecare Homebase (HCHB) experience High School Diploma or GED Attendance is mandatory for the first 90 days Home Health RN Case Manager Preferred Qualifications OASIS experience Wound care and wound vac management IV therapy and PICC line management Foley catheter care If you think this Home Health RN Case Manager position is a good fit for you, please reach out to me — feel free to call, e-mail, or apply to this posting!
