The Opportunity We are seeking an experienced Registered Nurse Case Manager to provide skilled home health services to patients across San Mateo and/or Santa Clara Counties. This is a flexible, field-based role with competitive compensation offered on a per visit or hourly basis, plus the option for full benefits. Responsibilities Conduct comprehensive patient assessments and develop individualized care plans Coordinate and manage all aspects of patient care across the care continuum Perform skilled nursing visits including wound care, medication management, and patient education Collaborate with physicians, therapists, and other care team members Ensure accurate and timely documentation in compliance with Medicare/Medicaid standards Monitor patient progress and adjust care plans accordingly Requirements Active California RN license Minimum 2 years of clinical nursing experience; home health experience preferred Strong knowledge of Medicare home health guidelines OASIS experience a plus Excellent time management and organizational skills Reliable transportation and valid California driver's license Compassionate, self-motivated, and comfortable working independently in the field Compensation & Benefits Competitive per visit or hourly pay Full benefits package available Flexible scheduling Supportive, team-oriented environment
