Your Daily Responsibilities Conduct thorough patient evaluations, analyze results from diagnostic tests, and craft tailored care plans. Diagnose and manage acute, episodic, or chronic conditions either independently or with physician collaboration (contingent on credentials). Guide patients and their families through education on health maintenance, wellness, and disease management. Prescribe necessary medications and treatment regimens in accordance with your professional scope. Engage in chronic care management and ample follow-up with patient cases. Work hand-in-hand with interdisciplinary teams to provide unified, comprehensive patient care. Maintain meticulous and accurate records using electronic medical record (EMR) systems. Educational Essentials Earned Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP). Graduated from an accredited Nurse Practitioner program. Mandatory Certifications Active Nurse Practitioner license in New York State. National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or an equivalent specialty. Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the ability to acquire them within 90 days).
