Position Overview You will deliver primary care and continuous healthcare services to PACE participants, utilizing your knowledge and skills to assess, plan, and provide care in alignment with PACE Program standards. You will work alongside the clinical care team to conduct assessments, develop care plans, and deliver comprehensive primary care. Key Responsibilities: Provide high-quality primary care focusing on individualized evaluation with comprehensive history and physical exams. Collaborate with the interdisciplinary team (IDT) before referring participants to specialists to ensure continuity of care. Work with the Medical Director to manage specialist referrals, initiate appropriate treatments, and communicate care plans. Manage medications and ensure thorough medication reconciliation aimed at exceeding expectations. Stay informed of participant status during hospitalizations, ensuring safe transitions and continuity of care. Share responsibility for all participants in the center, supporting a strong team-based care model. Participate in daily IDT meetings, offering clinical insights and collaborating on participant-centered care plans. Ensure accurate and timely completion of all participant assessments and documentation in compliance with standards. Review labs and consults promptly, updating care plans as necessary with the IDT. Educate participants and caregivers on self-care strategies, medications, and wellness practices. Use the Emergency Department judiciously, favoring PACE resources unless symptoms are life-threatening. Document diagnoses accurately to support risk adjustment and quality reporting metrics. Participate in an on-call rotation to address participant needs and ensure effective hand-offs and communication. Contribute to discharge planning to support transitions across care settings. Assist with staff training and mentoring, and contribute to clinical policy development. Engage in ongoing performance improvement initiatives. Demonstrate flexibility by supporting the team in other roles as needed. Communicate clinical resource needs to the Medical Director to support effective operations. Maintain compliance with all safety, privacy, and regulatory standards. Experience and Education: MD, DO, APRN, NP, or PA. Current State license. State CDS Certification and DEA Registration. Board Certified in Geriatrics preferred. BLS required (must be completed within 90 days of employment). 2 years of direct patient care experience preferred. 1 year of experience with elderly or frail populations preferred; training provided if needed. Pre-Employment Requirements: Must have reliable transportation, a valid driver's license, and minimum state-required liability auto insurance. Be medically cleared for communicable diseases and have all required immunizations up to date for direct participant contact. Pass a comprehensive criminal background check.
