What You'll Be Doing as a Nurse Practitioner/Physician Assistant - In-Home Primary Care (Field-BasedRole) Conduct in-home primary care visits, including comprehensive health assessments and chronic disease management. Develop and implement individualized care plans for adult patients with acute and chronic health conditions. Educate patients and families on health management strategies, medication adherence, and preventive care. Collaborate with a multidisciplinary team to ensure continuity of care and optimal patient outcomes. Document patient interactions using an EMR system (Athena preferred) to maintain accurate records. What You'll Bring as a Nurse Practitioner/Physician Assistant - In-Home Primary Care (Field-BasedRole) Certified Nurse Practitioner (NP) or Physician Assistant (PA) from an accredited program. Minimum of 2 years of clinical experience in primary care or home health, with a demonstrated ability to work independently. Active, unrestricted state license (or eligibility) and a valid driver's license. Current DEA registration (or eligibility) and Medicare/Medicaid enrollment (or eligibility). Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS). Proficiency with EMR systems and familiarity with Microsoft Office.
