This position is a hybrid opportunity in Riverside and requires the ability to work on-site with patient care (60%) and in case management (40%). What You Will Do Perform comprehensive health assessments including but not limited to the following: Obtain history and perform physical examination Complete review of medicines and related instructions Review home safety and psychosocial needs Fulfill documentation needs for STAR and HCC measures Obtain and document all pertinent information in patient's charts and case management notes on the same day the patient is seen; include medications, end-of-life preferences, and drivers of preventable hospital admissions Instruct all patients, caregivers, and family members to call prior to accessing medical care, subject to life-threatening emergencies Maintain patient safety and confidentiality Utilize universal precautions in all patient care Prescribe or order procedures, treatments, and medications within scope of practice Develop a high level of trust and intimacy with patients and families You Will Be Successful If Able to build strong, trusting relationships with patients and families Able to maintain patient safety and confidentiality while delivering high-quality care Able to collaborate in a team-based care environment and adapt quickly in a fast-growing, dynamic setting What You Will Bring CA Licensed Nurse Practitioner 1+ years of experience with Primary Care About Impresiv Health: Impresiv Health is a healthcare consulting partner specializing in clinical & operations management, enterprise project management, professional services, and software consulting services.
