See Education/Experience below.SUMMARY: Provides direct nursing care and supportive services to clients served by Friendship Community Care (FCC) and or the ICF within their scope of practice. This role supports continuity of care, promotes health, safety, and ensures services are delivered in compliance with applicable regulations, policies, procedures, and standards of care. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Assisting with implementation of care plans, administering medications and treatments as authorized, monitors and documents client's health status, and reports changes to supervise nurse or provider. Administers ordered medications and performed needed medical procedures as needed or determined by the supervisor-within RN Scope of Practice. Oversees client medication records once monthly in FCC facilities and as indicated by the Office of Long-Term Care (OLTC). Observes, monitors, and documents client health status, behaviors, and responses to care. Reports changes in condition, concerns, or incidents promptly to the supervisor or provider. Assist with care coordination, follow up, and implementation of health-related services in community settings. Maintains accurate, timely, and confidential documentation in compliance with HIPAA and organizational policies and procedures. Supports health education for clients and caregivers. Follows infection control, safety, and emergency procedures. Collaborates with interdisciplinary team members to support client well-being and service delivery. Performs regular nail care on diabetic patients and will maintain a log and document via a Nurse's Note in the EHR. Provides assistance, guidance, and oversight to clients and support staff with personal hygiene needs, including bathing, grooming, nail care, and hair care, as needed and in accordance with care plans. Assists with on-the-job training of new employees regarding medication and first aid. Assists with surveillance of the residential facilities and waiver homes to ensure compliance with health and safety policies and procedures Monitors recently hospitalized or newly admitted clients in accordance with FCC procedure. Exhibits proper cooperation, enthusiasm, and interest in all areas necessary for success for all FCC and associated services provided. Performs dressing changes as ordered by a physician. Reviews and updates MAR with any medication/treatment changes ordered as needed. Provides routine medical information to parents/guardians as requested. Coordinates medical and health related information with nursing staff and case management. Maintains psychiatric clinic records and coordinates clinic scheduling, as needed. Follows up on assigned nursing tasks and orders. Performs admissions as directed by the RN supervisor. Participates in conferences and in service training as required. Notify day programs of any medication changes that will affect their medication pass. Request medication clarification as needed. Document and follow up on MARKS reports as indicated. Handle safety checks at various facilities as assigned. Report to facility administrator (or designee), any information and/or incidents reportable under OLTC guidelines. Rotate with other members of the nursing department for on call as scheduled. Complete on-call log and report it to Director of Nursing. Works with other nurses to provide coverage for vacations and/or illness of any nurse in FCC. Serve as a driver for client day trips to various state locations. Driving may be required on as much as a daily basis. Required to undergo periodic driving test and demonstrate proficiency in the operation of a motor vehicle, including passenger vans. Employee may also be required to drive his/her own personal vehicle at times and maintain adequate liability coverage on any personal vehicle used to transport clients. Consumers' progress notes be maintained and completed daily or as needed on the prescribed forms. Consumers' records must be maintained weekly, monthly, quarterly, and yearly. All information must be filed in consumers' records as needed and updated as prescribed by funding sources. EDUCATION and/or EXPERIENCE: High School Diploma or General Education Degree (GED), required. Registered Nursing Degree from an accredited college or technical school. Must hold a current Arkansas RN license and be in good standing with the Arkansas State Board of Nursing (ASBN). Experience in Community Based Care, LTC, or similar setting preferred. Current CPR and First Aid Certification. Must meet clearance specifications through OLTC Registry Check System. Proficiency in computer applications and basic software is required. LANGUAGE SKILLS: Ability to read, understand, and follow written and verbal instructions, medical orders, policies, and procedures. Communicates clearly and professionally with clients, families, interdisciplinary team members, and external providers in one-on-one and small group settings. Able to document accurately and legibly in medical records and complete basic written correspondence related to client care and services. MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide in all units of measure using whole numbers, common fractions, and decimals. Ability to calculate rates, ratios, and percentages and to create and interpret bar graphs. REASONING ABILITY: Ability to apply common-sense reasoning to follow instructions provided in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
